Our Services
Community First Services and Supports (CFSS)
GentleCare Home Health LLC provides Community First Services and Supports (CFSS) for
eligible individuals who choose the CFSS agency model. CFSS is designed to help individuals
remain in their homes and communities while receiving support with daily activities and
approved care needs.
What CFSS May Include
Support workers may assist with daily living activities, bathing, dressing, grooming, mobility
assistance, health-related activities, behavior observation and monitoring, and general
day-to-day support.
How Our Agency Supports You
As a CFSS provider agency, GentleCare Home Health LLC helps by recruiting support workers, hiring workers, training workers, supervising workers, and evaluating workers. The individual receiving services still plays an important role in directing care, choosing workers, helping set schedules, and participating in training or supervision decisions
Person-Centered Support
We believe individuals should have a meaningful say in how their care is delivered. Our role is to support that process with reliable staffing, organized communication, and professional service.
Payment and Funding
Eligible funding sources may include Medical Assistance (MA), Alternative Care (AC), HCBS
Waivers, and MinnesotaCare for some eligible individuals.
